Diabetes Education
WHF offers a diabetes education program that provides weekly nutrition and physical education groups for low-income community members who are diabetic, at risk for diabetes, or who live with a diabetic family member. The groups provide support for the participants, as well as a venue for learning more about their own health care. The nutrition groups provide hands-on cooking demonstrations, food tasting, and share healthy recipes. The physical education groups teach exercises and provide time for the participants to be active. The progress of the participants is measured every three months via physical measurements, lab tests, and a one-on-one progress survey.
In addition to the diabetes educational groups, diabetic patients at WHF also have an opportunity to participate in group medical visits (offered in Spanish and English). These groups are lead by a primary care physician and a medical assistant/health educator. Promotoras often provide additional support during these groups. Medical group visits provide excellent case management and follow up for the patients, as well as providing a venue for support and encouragement.
