Older Adults

Winters has always maintained an honored place for its older citizens and we recognize that the senior patients in our practice represent lifetimes of experience and wisdom. The health care needs of older adults are unique and complex. It takes special skill and experience to manage their many medical and social issues. WHF physicians and care providers offer chronic and urgent care services to geriatrics as well as prevention screening for acute and chronic conditions.

At a clinic visit, patients are asked for a complete medical history and receive physical and psychosocial assessment. The patient is encouraged to return for regular preventive visits. Geriatric patients will have a medication list and updated immunizations maintained in the WHF electronic medical records system. Our trained staff will refer patients for outreach and health education and to other social services agencies as appropriate.

Many families in Winters strive to keep older family members at home where they are happiest and most healthy. Winters Healthcare is committed to helping families keep their loved ones at home. For older adults who have difficulty getting to the clinic, we have a home care practice where physicians and nurse case managers provide primary care and case management in the patient's home. We also provide care to patients living in local nursing homes.
