We operate a primary care medical office that provides healthcare to individuals of all ages, from pediatrics to elder care through an integrated team approach with all of our staff working together and alongside a Behavioral Health Consultant and health coaches. In addition to the medical office we have a dental team providing education, support and services for general dentistry. In addition to our medical and dental services, we do community outreach, host nutrition and exercise classes and we have Certified Enrollment Counselors on staff for enrollment assistance.
We are a small organization, and we treasure and nurture the relationships between our patients and providers. We believe in a team based approach as well as relationship based model of care with our patients to partner with them on their health journey. Our 40 staff members serve more than 3,000 patients. We actively work with other groups in and around Winters, CA to provide a myriad of services and supports to our patients and to improve the health of our entire community.
